Student Health Insurance Applications and Brochures:
Students may view or print a variety of information. If you decide to print be sure
to print all pages of the information you need. You may enroll using the appropriate
application. Students may enroll online, or mail the signed application, with a
check, to the address located in the application instructions. GAs must enroll online.
If you need further assistance or have enrollment questions, please contact the
Student Insurance Group directly at 1-866-931-9560.
Note: Visiting research scholars and English Language Institute students
may also purchase this student insurance. International Students, who are not GAs,
must contact the USF SHS Student Insurance Office directly.
Be sure to include your Apartment or Unit number (if applicable). Providing an
incomplete or invalid address will delay your enrollment/re-enrollment, and will
also cause a delay in the mailing of your insurance card.
